Do It Yourself Illinois NCLEX Application for Filipino Nurses

We, Filipino nurses, are not well compensated back in our country. Many of us struggle to pay the processing agency to work on our papers just to take the NCLEX. To help you, I have made a step by step DIY process for the state of Illinois.

Remember, now is the best time to get the USRN title and then process your visa application after getting that license.

Five general steps to complete the application:

CGFNS ($365)

·      Open CGFNS account and register.

·        Fill up necessary information and attach a 2x2 picture.

 

    ·   Go to Shopping Cart, order Credential Evaluation Service (CES) Professional Report, choose IDFPR as the recipient and pay 365 dollars

1. PRC License Validation (150pesos)

  ·       Download and print CGFNS license validation, fill up necessary information and indicate your signature.

     ·  Submit it to PRC together with a photocopy of your transcript of records (TOR) and pay 150 pesos at the cashier

·      Have PRC mail it to CGFNS International in Philadelphia, USA through FeDex and obtain the tracking number. 

           

 2. Transcript of Records

·    Download and print CGFNS Request for Academic Records, fill up necessary information and indicate your signature

     ·   Go to your School registrar office for a request of  the Transcript of Records, thereafter, submit the CGFNS form to the department of nursing for them to fill up the academic records. Once all documents are gathered, have it sealed and stamped by the registrar/university and don’t open it.

      ·     Send the sealed document to CGFNS International in Philadelphia, USA using your school’s                  name and address.

*Note: CGFNS will not accept your document if you will use your name as the sender.

3. Secondary/HS Diploma

     ·    Download and Print CGFNS Request for Secondary School Diploma, indicate your signature. 

     ·  Once signed, scan both CGFNS form and HS diploma, save it in one PDF file.

     ·   Email it to CGFNS through Contact us page of your CGFNS Account.

If you have submitted all documents and CGFNS approved your application then the status on your dashboard will change to “Report Issued”

 

Fingerprinting ($68)

Only obtain and submit your fingerprinting application once CGFNS approve your CES Report.

·  You may visit this link for Illinois based vendors for fingerprinting: http://www.idfpr.com/LicenseLookUp/fingerprintlist.asp

·        In my case, I processed mine through “A Fingerprinting U S Photo Inc.”

·       You may visit their website to download and print the fingerprint form.

·    Obtain 1 or 2 copies of fingerprint card. Thereafter, go to National Bureau of Investigation (NBI) to have your self-fingerprinted by a law enforcer.

·     Mail the said documents to your chosen vendor with your credit card payment details.

·      You will receive a receipt via email and send a copy of it to Continental Testing Services Inc. together with the application form, CCA and 98 dollars bank draft.

You can start your application to the Illinois Board of Nursing through Continental Testing Service Inc (CTS) once CGFNS approve your CES Professional report and the status indicated “report issued”.

·        Open CTS website with this link:

·         Click “Apply for Exam” then input the necessary information for “application file” and “CCA file”

        

    ·     Use “999-99-999” for the social security number and select “all other applicants”. After, press start to fill up identifying information.

  ·        Download the file sent by CTS on your email. Print and affix your signature on the applicant and CCA form.

    ·   Thereafter, mail the following documents: 

ü  CCA Form with signature and correct information

ü  Fingerprint form and receipt

ü Printed Copy  of CGFNS Report

ü   98 dollars Bank draft/ Money draft addressed to “Continental Testing Service Inc.”

Mail all DOCUMENTS to:

Pearson Vue Registration ($200)

·  Start your registration and 200 dollars payment to Pearson Vue after sending all the remaining documents to Continental Testing Service Inc.

    ·  CTS will coordinate to Pearson Vue for your Authorization to Take Test (ATT).

    

    · You will receive an email from Pearson Vue of your ATT. Then, you may now schedule your                     NCLEX Exam.

Scheduling ($150)

    ·   If you will take your NCLEX outside the United States of America, you need to pay 150             dollars by the time you schedule your exam.

You have now successfully completed the process.
